Anthem Broadband understands that privacy online is important to users of the Anthem Broadband website (the “Site”), especially when conducting business. This Privacy Policy governs our practices with respect to both those users who visit the Site without transacting business (“Visitors”) and those users who register to transact business on the Site and make use of the various services offered by Anthem Broadband (collectively, the “Services”) (“Customers”).

What is “Personally Identifiable Information”?

Personally Identifiable Information refers to any information that identifies or can be used to identify, contact, or locate the person to whom such information pertains, including, but not limited to, name, address, phone number, fax number, email address, financial profiles, social security number, and credit card information. Personally Identifiable Information does not include information that is collected anonymously (that is, without identification of the individual user) or demographic information not connected to an identified individual.

What Personally Identifiable Information is collected?

Anthem Broadband may collect basic user profile information from all of our Visitors and Customers Anthem Broadband also may collect the following additional information from our Customers: names, addresses, phone numbers, and email addresses of Customers, the nature and size of the business, and the nature of the service the Customer intends to purchase.

What organizations are collecting the information?

In addition to Anthem Broadband’s direct collection of information, third party service vendors (such as credit card companies, clearinghouses and banks) who may provide such services as credit, insurance, and escrow services may collect information from Visitors and Customers. Anthem Broadband does not control how these third parties use such information, but does ask them to disclose how they use personal information provided to them from Visitors and Customers. Some of these third parties may be intermediaries that act solely as links in the distribution chain, and do not store, retain, or use the information given to them.

How Anthem Broadband Collects Your Information

How does Anthem Broadband use the information?

Anthem Broadband uses Personally Identifiable Information to customize the Site, to make appropriate service offerings, and to fulfill buying and selling requests on the Site. Anthem Broadband may email Visitors and Customers about research or purchase and selling opportunities on the Site or information related to the subject matter of the Site. Anthem Broadband may use Personally Identifiable Information to contact Visitors and Customers in response to specific inquiries, or to provide requested information. Anthem Broadband also uses Personally Identifiable Information to maintain accounts and provide and repair the Services. As a provider of Voice over Internet Protocol services, Anthem Broadband relies on Customers to keep their service addresses current when they change locations, as Anthem Broadband may need to utilize such information in the event of an emergency.

With whom may the information may be shared?

Personally Identifiable Information about Customers may be shared with other Customers who wish to evaluate potential services with other Customers. Anthem Broadband may share aggregated information about Visitors, including the demographics of Visitors and Customers, with our affiliated agencies and

third-party vendors. Anthem Broadband also offers the opportunity to “opt out” of receiving information or being contacted by Anthem Broadband or by any agency acting on its behalf.

How is the information stored?

Personally Identifiable Information collected by Anthem Broadband is securely stored and is not accessible to third parties or employees of Anthem Broadband except for use as indicated above.

What choices are available regarding collection, use, and distribution of the information?

Visitors and Customers may opt out of receiving unsolicited information from or being contacted by Anthem Broadband and/or its vendors and affiliated agencies by responding to emails as instructed, or by contacting us at 906 S Oneida St, Suite 8 Rupert, ID 83350.

Are Cookies Used on the Site?

Cookies are used for a variety of reasons. Anthem Broadband uses Cookies to obtain information about the preferences of our Visitors and the services they select. We also use Cookies for security purposes to protect our Customers. For example, if a Customer is logged on and the Site is unused for more than 10 minutes, Anthem Broadband can automatically log the Customer off.

How does Anthem Broadband use login information?

Anthem Broadband uses login information, including, but not limited to, IP addresses, ISPs, and browser types, to analyze trends, administer the Site, track a user’s movement and use, and gather broad demographic information.

What third parties have access to information from Visitors and/or Customers on the Site?

Anthem Broadband has entered into and will continue to enter into partnerships and other affiliations with a number of vendors. Such vendors may have access to certain Personally Identifiable Information on a need-to-know basis for evaluating Customers’ eligibility for Services. This Privacy Policy does not cover such vendors’ collection or use of Personally Identifiable Information. Anthem Broadband will disclose Personally Identifiable Information to third parties in order to comply with a court order or subpoena or a request from a law enforcement agency to release information. Anthem Broadband will also disclose Personally Identifiable Information when reasonably necessary to protect the safety of Visitors and Customers.

How does the Site keep the information secure?

All of Anthem Broadband’s employees are familiar with its security policy and practices. The Personally Identifiable Information of our Visitors and Customers is password-protected and only accessible by a limited number of qualified employees. Anthem Broadband audits its security systems and processes on a regular basis. Sensitive information, such as credit card numbers or social security numbers, is protected by encryption protocols, in place to protect information sent over the Internet. While Anthem Broadband takes commercially reasonable measures to maintain a secure Site, electronic communications and databases are subject to errors, tampering and break-ins, and Anthem Broadband cannot guarantee or warrant that such events will not take place. Anthem Broadband will not be liable to Visitors or Customers for any such occurrences.

What does Anthem Broadband do if there is an unauthorized disclosure of information or a security breach? Despite Anthem Broadband’s best efforts to protect Personally Identifiable Information, no program is 100% secure and Anthem Broadband cannot guarantee its safeguards will prevent every unauthorized access, use, or disclosure of personal information. A number of other laws may require Anthem Broadband to notify individuals or state agencies of certain kinds of unauthorized disclosures. Anthem Broadband will examine all applicable laws and comply with any disclosure requirements. If you think information about you or others has been disclosed without proper authorization, please contact Anthem Broadband.

How can Visitors correct any inaccuracies in information?

Visitors and Customers may contact Anthem Broadband to update Personally Identifiable Information about them or to correct any inaccuracies by emailing Anthem Broadband at billing@anthembroadband.com.

Can a Visitor delete or deactivate information collected by the Site?

Anthem Broadband provides Visitors and Customers with a mechanism to delete/deactivate Personally Identifiable Information from the Site’s database. However, because of backups and records of deletions, it may be impossible to delete a Visitor’s entry without retaining some residual information. An individual who requests to have Personally Identifiable Information deactivated will have this information functionally deleted, and Anthem Broadband will not sell, transfer, or use Personally Identifiable Information relating to that individual in any way moving forward.

What happens if the Privacy Policy Changes?

Anthem Broadband will let Visitors and Customers know about changes to this Privacy Policy by posting such changes on the Site. Visitors and Customers will be deemed to have notice of changes to the Privacy Policy upon Anthem Broadband’s posting of an updated policy on the Site. However, if Anthem Broadband is changing this Privacy Policy in a manner that might cause disclosure of Personally Identifiable Information that a Visitor or Customer has previously requested not be disclosed, we will contact such Visitor or Customer to allow such Visitor or Customer to prevent such disclosure.

VoIP

Anthem Broadband acts as a data processor with regard to indirect end user personal identifiable information and our Clients act as the data controller of such data. In the course of Anthem Broadbands' processing and protection of such data, all use will be in conformity with the data controller’s instructions.

Specifically, only when enabled via system permission by the user on the Anthem Broadband VoIP Android and Anthem Broadband VoIP iOS applications, Anthem Broadband shows personal contacts within the respective application. When the user sends an SMS message to one of his/her phone contacts, or when the user initiates a call to one of his/her phone contacts, the phone number is sent securely through the Anthem Broadband API. Anthem Broadband does not store this number with any other Customer Properitary Network Information (CPNI), and it cannot be directly or indirectly attributed to any person or persons; Anthem Broadband stores only the phone number and pertinent metadata so as to be compliant with all applicable state and federal laws, and Anthem Broadband does not share this data with any advertisers or third parties under any circumstances. A user can revoke phone contact access on his/her mobile device at any time, and his/her app experience is not hindered or interrupted.

Anthem Broadband VoIP Android and iOS applications also use Gravatar, only when enabled via Settings, a service that provides avatar images linked to the MD5 hash of the user’s email address. This means that, only when Gravatar use is enabled, we hash each contact’s email address and send it to Gravatar to try and retrieve an avatar image. MD5 hashes cannot be directly or indirectly attributed to any person or persons, and we only send the MD5 hash to Gravatar, never the email address in plain text. As with phone contacts, a user can revoke Gravatar access at any time in Settings and his/her app experience is not hindered or interrupted.
